NRL star who tragically died having collapsed after intensive training session regained consciousness while in the ambulance – but doctors still don't know how he died

A rising NRL star and 'gentle giant' who collapsed after a 90 minute training session temporarily regained consciousness before he died - as his older brother says he was 'doing what he loved'.

Manly Sea Eagles up-and-comer Keith Titmuss died on Monday after completing a 90-minute non-contact workout in Narrabeen.

The 20-year-old was stretching post-training and is said to have complained that he was cramping, before he collapsed and was rushed to hospital.

According to rugby league journalist Adam Pengilly, Titmuss regained consciousness and even appeared 'lucid' before his condition deteriorated again.

'This is the strange thing for me, after he left Narrabeen he sort of regained consciousness and he was lucid to some extent,' he said on radio on Tuesday.

'And unfortunately the situation deteriorated quite rapidly once he got to hospital. And it's just a tragic, tragic story.'

Pengilly said his cause of death was still unknown. 

'No one knows what the cause is,' he said. 'A lot more will come to light in coming days and coming months.'

Manly had completed a non-contact conditioning and fitness session before his untimely death. 

Titmuss had just been promoted to the top-30 squad after impressing coaching staff for several years in the Jersey Flegg Cup and lower grade competitions. 

His brother Jesse shared a touching tribute to the 20-year-old on Tuesday.

'Keith passed on doing what he loved, living out his dream and there is no doubt about that. The hardest working individual I knew, with a mindset like no other,' he said.

Jesse commended his younger brother's 'never give up mentality' and said it was his drive and ambition that got him ahead in life.

'If anyone knows me, Keith was all I would talk about,' he said. 'My rock, my best friend, you were always by side through thick and thin. I miss you so much.'

Titmuss was 'a man of very few words' but left an impact on everybody he met, Jesse said.

While football was his passion, family and friends were always 'the most important' parts of his life and he often thought deeply about any advice offered to him. 

Jesse said his brother was a 'loving, gentle giant' who was looking forward to what could have been his breakthrough season in the NRL. 

He felt fit and healthy just days prior to his death, telling his older brother he'd done everything he could to be prepared for Manly's pre-season.

The team's chief executive Stephen Humphreys said the young star didn't have any known medical conditions, and the training session had been non-contact. 

'We are also without answers about what has happened,' he told the Sydney Morning Herald, saying the club was 'in shock'. 

Sea Eagles Coach Hasler said the club lost a member of its family.

'We are all devastated by this news,' Hasler said in a statement.

'Keith was a very popular character among the playing group. He will be sorely missed but never forgotten by the Sea Eagles.' 

Mr Humphreys said the club was supporting the players as well as the rising star's family.

'Our Club sends its deepest condolences to Keith's family and friends and will provide them with all of the support they need during this difficult period,' He said.

'We are working with the NRL's Wellbeing team to offer support and counselling to our playing group and staff.'

Andrew Abdo, NRL's chief executive, said it was a 'tragic day for rugby league'.  

'The game has lost a promising young player with the world at his feet,' he said.

'We are one family and today we have lost a member of that family. On behalf of the Commission and the game I send my deepest condolences to Keith's family.' 

The forward player made an impression after scoring the match-winning try for the Sea Eagles in the 2017 NYC Grand Final.

He was also named the Manly Jersey Flegg Cup Players' Player in 2019.
