A search is underway for a missing 12-year-old girl who was last spoken to at her home five days ago.

Jiavanti Prasad was last seen by a family member on December 3 before she left for school in the St George region, south of Sydney's CBD.

However, she never made it to school that day and hasn't attended since.

She was reported missing on Monday, after police made brief contact with her before she vanished again.

Jiavanti often hangs out in suburbs like Bexley, Hurstville, Maroubra and Mount Druitt.

Attempts have been made to contact Jiavanti through friends and family.

However, they have been unsuccessful.

NSW Police have conducted an extensive search since they were notified.

Authorities hold serious concerns for Jiavanti's welfare due to her age.

Anyone with information is urged to contact Crime Stoppers, 1800 333 000 or St George Police on 02 8566 7499.
