Another traveller managed to dodge hotel quarantine in Sydney and get on another flight after arriving from the US - but this time NONE of passengers on the plane to Melbourne were told

The Victorian Government told no one of another hotel quarantine breach while it was struggling to contain a deadly second wave of Covid-19.

An aviation worker flew into Sydney from San Francisco before taking a domestic flight into Melbourne on July 14, skipping the mandatory 14 day quarantine.

Airline staff identified the breach and he was placed into quarantine at the Park Royal airport hotel where he tested negative for Covid-19.

However, unlike the case of two German-Australians last week, health authorities did not notify anyone on the flight to Melbourne, and the breach was not made public.

The non-compliance was leaked to The Age after reports emerged at the weekend about the mother and son breaching quarantine measures.

The 53-year-old woman and her 15-year-old son were directed onto a domestic flight to Melbourne after touching down in Sydney from Tokyo on Saturday. 

A NSW Police officer mistakenly believed they had a hotel quarantine exemption and failed to thoroughly check their paperwork.

But unlike the weekend's debacle, the incident back in July was not revealed to the public.

The man worked for the regional American airline ExpressJet but was travelling into Sydney as a passenger on United Airlines flight UA863.

Domestic airline staff detected the breach and the airline worker was detained and placed into hotel quarantine at Melbourne Airport.  

The Victorian Department of Health said it was not procedure to notify potential contacts at the time. 

A spokesman told the Age the policy of isolating secondary contacts was introduced three months later in October.

'We now contact trace both primary close and secondary close contacts,' he said.

But the Victorian President of the Australian Medical Association Julian Rait said the incident should have been reported, so staff could have learned from it.

'The best healthcare institutions have an open disclosure policy where not only are patients told of any unexpected mistake, but staff are encouraged to reveal them and examine how they might be prevented in the future,' he told the Age.

'That applies equally as much to NSW as Victoria, it's easy to be critical but I think we all need to reflect on what's the most appropriate organisational culture that will lead to better outcomes.'

Mr Rait said the situation points to 'a lack of an adequate safety culture'.  

The man tested negative for Covid-19 on day three and day 11 of his stay.

NSW police could not find any issues in their paperwork for passengers arriving into Sydney on this date.

A spokesperson said air cabin crew would not have contact with police as they are processed separately to regular passengers.

NSW police accepted blame for the quarantine breach on Saturday.  

International flights only began operating to Melbourne on Monday after a second wave triggered a ban on international arrivals on July 10.

But at that stage the number of active cases for Covid-19 had grown to 1,172.

Meanwhile the Victorian Government wants to see other states adopt the processes it has in place for quarantine arrangements at Melbourne airport.

All international arrivals are kept separate all at times rather than being allowed into general areas of the airport.
