London bus driver, 58, is among three Covid patients to die at capital's NHS Nightingale Hospital after ICU staff failed to plug-in filter in ventilator that kept his airway clear - as report shows 10 people were affected by fault

Three Covid patients at London's NHS Nightingale Hospital died after being treated by 'inexperienced' staff using the wrong ventilator equipment during the first wave of the pandemic.  A total of ten patients were affected at the hospital in the ExCel Centre, Newham, East London, after staff left their intensive care ventilators without heat and moisture exchange filters. HME filters are  placed at the patient's end of the breathing system in order to humidify the  airway in the machine and prevent the build of mucus, which in turn can block a patient's airways and endotracheal tube.   But the staff error, which saw them use the wrong filter in the ventilation circuits instead, led to the breathing tubes in three patients becoming blocked and saw all of them 'suffer harm' and require re-intubation.  All three patients died after the incident occurred.  The findings are among a catalogue of issues that have emerged since the Nightingale, which was hailed as

T-Mobile CONFIRMS it has been hacked amid claims 100m customers' social security numbers, driver licenses and personal details were stolen and are on sale for $270,000 worth of Bitcoin

T-Mobile has confirmed its servers have been hacked - but refused to confirm claims 100 million customers personal data - including social security numbers and drivers licenses - are now for sale online.  The hack was confirmed Monday afternoon, with a spokesman saying: 'We have determined that unauthorized access to some T-Mobile data occurred, however we have not yet determined that there is any personal customer data involved.' T-Mobile offered no further details of the breach, or confirmation that details including the names, addresses and unique phone serial numbers - known as IMEIs - of almost its entire customer base had been compromised. Cyberscoop reporter Tonya Riley tweeted that the telecoms giant had told her it was confident it had sealed-off the security loophole that had allowed the hack to happen, and that it was working with law enforcement to investigate the crime.   The unconfirmed allegations about what exactly the hack involved could mean nearly all T-Mobil

Delay I fear is their death sentence: For years, DAVID WILLIAMS has led the charge to give sanctuary to brave Afghan translators. Now, he shares his fears and fury that help has come too late for too many... and the red tape is a noose around their necks

The phone at my bedside buzzed all through the night, and sleep was fitful. There were 142 messages: each a tale of human misery, illustrating the desperate plight facing the brave Afghans who helped Britain in its 20-year mission, but who now fear the worst after the vengeful Taliban recaptured the country. Take Farid. When I reached him yesterday, his sobbing was uncontrolled and heartbreaking. It was not for himself that the former British Embassy interpreter wept, but for his wife and children. As the insurgents walked triumphantly into Kabul 24 hours earlier, Farid, 37, had received threatening calls. He moved his family to a relative’s home, thus escaping five suspected Taliban who arrived at his house asking for him by name. For now. ‘If they find me, they will kill me — our enemy does not suddenly become our friend,’ he says. ‘If I die because of my work for the British that is one thing, but what about my wife and children? Will they all die as a consequence of my work? Afghan
